Companies such as Johnson &amp; Johnson currently sell infant versions of the drugs (80 milligrams) that contain half the amount of acetaminophen found in regular children&#8217;s formulas, the news service said.</p>
<p>Acetaminophen is a widely used drug found in a variety of products that helps to ease pain and reduce fever. Although it&#8217;s usually safe if used according to directions, too much can damage the liver. Acetaminophen overdose is the leading cause of liver failure in the United States, with more than 50,000 people requiring emergency-room treatment each year, the AP said.</p>
<p>The manufacturers&#8217; announcement preceded a U.S. Food and Drug Administration meeting scheduled for later this month, at which time agency advisers will consider whether clearer instructions and safety labeling are needed for products containing acetaminophen that are marketed for children younger than 2 years of age, the news service said.</p>
<p>Earlier Wednesday, the FDA issued final guidance for the production, marketing and distribution of liquid over-the-counter <a href="http://www.drugs-prescription.org">drug products</a> that are measured and dispensed with provided devices such as spoons, cups and droppers.</p>
<p>The agency developed the guidance in response to concerns about the risk of overdoses when using liquid over-the-counter (OTC) pain relievers, cold medicines, cough syrups and digestion aids if the dispensing devices included with the products have markings that are confusing, unclear or inconsistent with the dosage directions on the label.</p>
<p>Among the main recommendations in the guidance:</p>
<p>* Dosage dispensing devices should be included with all OTC liquid drug products that are taken by mouth.<br />
* The dispensing devices should be marked with calibrated units of liquid measurement (such as teaspoon, tablespoon, or milliliter) that match the units of measurement specified on the label directions. The devices should not have any unnecessary markings.<br />
* Companies should ensure that dispensing devices are used only with the intended products.<br />
* The measure markings on dispensing devices should be clearly visible when the liquid product is added to the device.</p>
<p>&#8220;Accidental medication overdose in young children is an increasingly common, but preventable, public health problem,&#8221; Dr. Karen Weiss, program director for the Center for Drug Evaluation and Research&#8217;s Safe Use Initiative, said in an FDA news release.</p>

<p>The trend, reported in the December issue of Pediatrics, mirrors a similar increase in misuse of these drugs, with adolescents and young adults&#8217; illicit use of <a href="http://www.drugsprescription.com">prescription drugs</a> now outstripping all other illicit drug use except marijuana.</p>
<p>The researchers couldn&#8217;t attribute the increased misuse directly to more prescriptions, but did urge both physicians and patients to be vigilant when considering the use of drugs such as Oxycontin or Ritalin.</p>
<p>&#8220;Our study did not look at the relationship between prescribing and misuse, but the increased prescribing increases the potential availability [of these drugs],&#8221; said study author Dr. Robert Fortuna, an assistant professor of internal medicine and pediatrics at the University of Rochester Medical Center. &#8220;Physicians and patients need to be aware of the increased rates of prescribing, be aware of misuse and have discussions about the risks and benefits.&#8221;</p>
<p>&#8220;In total, a controlled medication was prescribed at approximately one out of 6 [health-care visits] for young adults and one in nine for adolescents,&#8221; he added. &#8220;The numbers are large.&#8221;</p>
<p>Increased prescribing rates for <a href="http://www.nordmed.com">controlled medications</a> is not a new phenomenon, and many believe that some of it has to do with recent initiatives to make sure pain isn&#8217;t undertreated.</p>
<p>&#8220;We believe we&#8217;ve been underusing <a href="http://www.painreliefmedicine.com">pain medicine</a> and sedative-hypnotic medications because we have been so concerned about the potential for abuse,&#8221; said Dr. Jess P. Shatkin, director of education and training at the NYU Child Study Center in New York City.</p>
<p>&#8220;Increases are possibly due to changing regulations at both the federal and state levels,&#8221; Fortuna agreed. &#8220;There has been an increased advocacy to treat patients for pain, and there&#8217;s some evidence that physicians are becoming increasingly comfortable prescribing these medications.&#8221;</p>
<p>Opioids are a prime example, with sales of oxycodone surging 732 percent and those of methadone by more than 1,000 percent between 1997 and 2006.</p>
<p>The authors looked at two national databases to determine how often opioids, sedative-hypnotics and stimulants were prescribed to adolescents and young adults.</p>
<p>In 2007 alone, a scrip for one of these medications was written at 2.3 million visits by adolescents and 7.8 million visits by young adults.</p>
<p>In 1994, such medications were prescribed at only 6.4 percent of such visits for adolescents, but rose to 11.2 percent in 2007. For young adults, the rate increased from 8.3 percent to 16.1 percent.</p>
<p>Doctors wrote these prescriptions in regular doctors&#8217; offices and in emergency departments, for injuries and for more pedestrian complaints such as back pain.</p>
<p>A drawback of the study, Fortuna conceded, was that it &#8220;did not look at diverted or misused medications.&#8221;</p>
<p>He and the other researchers called for more investigation into prescribing trends and prescription misuse, noting the potential for trauma, high-risk behaviors and unintentional overdose among those abusing controlled substances.</p>

<p>&#8220;There&#8217;s a potential connection between the more prescriptions you have going on and the more potential risk that patients will use it themselves for a purpose it wasn&#8217;t it intended for or divert it to a friend or someone else,&#8221; said Shatkin. &#8220;It&#8217;s not infrequent that that happens.&#8221;</p>
<p>&#8220;We certainly have a concern about increased misuse and increased diversion,&#8221; said Fortuna. &#8220;The increase in prescribing of controlled medications such as opioids, stimulants and sedatives in and of itself is not necessarily bad as long as they&#8217;re being used appropriately.&#8221;</p>

			<content:encoded><![CDATA[<p>Substance abuse treatment admissions for non-medical use of <a href="http://www.orderrxpharmacy.com/pain.html">prescription pain relievers</a> have increased more than 400 percent over 10 years, according to a new government study released Thursday.</p>
<p>The proportion of admissions for people age 12 and older increased from 2.2 percent in 1998 to 9.8 percent in 2008, and this rise was seen in all age, gender, race and ethnic groups, as well as in people of all education levels and in all regions of the country, the Substance Abuse and Mental Health Services Administration (SAMHSA) reported.</p>
<p>&#8220;The non-medical use of <a href="http://www.onlinepharmacy.ws/pain.html">prescription pain relievers</a> is now the second most prevalent form of illicit drug use in the nation, and its tragic consequences are seen in substance abuse treatment centers and hospital emergency departments throughout our nation,&#8221; SAMHSA Administrator Pamela S. Hyde said in an agency news release.</p>
<p>&#8220;This public health threat demands that we follow the President&#8217;s National Drug Control Strategy&#8217;s call for an all-out effort to raise awareness of this risk and the critical importance of properly using, storing, and disposing of these powerful drugs,&#8221; she added.</p>
<p>The study, based on data from SAMHSA&#8217;s Treatment Episode Data Set, a reporting system involving treatment facilities from across the country, showed that the proportion of admission rates increased from 1.8 percent to 8.1 percent for men and from 3.5 percent to 13.3 percent for women. Rates increased from 1.9 percent to 9.7 percent for those with an eighth-grade education or less, and from 3.8 percent to 12.1 percent for those with more than a high school education.</p>
<p>&#8220;Our national <a href="http://www.drugs-prescription.org">prescription drug</a> abuse problem cannot be ignored,&#8221; A. Thomas McLellan, deputy director of National Drug Control Policy, said in the news release. &#8220;I have worked in the treatment field for the last 35 years, and recent trends regarding the extent of prescription drug abuse are startling. We must work with prescribers, the pharmaceutical industry, and families to help us fight this scourge.&#8221;</p>

<p>Noting that pain and depression often occur at the same time, the current observation blends two competing schools of thought, in which some believe that pain is &#8220;all in the head&#8221; while others contend that pain is &#8220;all in the body.&#8221;</p>
<p>To see how pain and depression might intersect, the research team &#8212; led by the University of Oxford&#8217;s Dr. Chantal Berna &#8212; used brain imaging to conduct pain tests on healthy participants who were induced to feel sad.</p>
<p>A depressed mood appeared to affect brain nerve circuitry responsible for emotion, resulting in a stronger perception of pain, according to the report published in the June 1 issue of Biological Psychiatry.</p>
<p>&#8220;When the healthy people were made sad by negative thoughts and depressing music, we found that their brains processed pain more emotionally, which lead to them finding the pain more unpleasant,&#8221; Berna said in a news release from the journal&#8217;s publisher.</p>
<p>Berna&#8217;s team theorized that one&#8217;s ability to control the negative emotions linked to pain are short-circuited by depression, leading to a bigger punch when pain hits. In other words, depression may not only be a consequence of being in pain. It might actually exacerbate pain, making it worse than it would be for those in a positive frame of mind.</p>
<p>&#8220;Our research suggests depressed mood leads to maladaptive changes in brain function associated with pain, and that depressed mood itself could be a target for treatment by <a href="http://www.orderrxpharmacy.com/antidepression.html">depression medicines</a> or psychotherapy in this context,&#8221; Berna explained in the news release.</p>
<p>Following this line of research, the next step would be to study patients with chronic pain, because they often also suffer from depression, the researchers noted. The goal would be to find ways to more effectively treat the millions of people worldwide who experience chronic pain and depression, the authors explained.</p>

			<content:encoded><![CDATA[<p>People who are uninsured or who are on Medicaid are much less likely to receive adequate care for their migraines than people who have private coverage.</p>
<p>In fact, more than 5.5 million Americans may be at risk of receiving inadequate care for their migraines because of their insurance status, a new study suggests.</p>
<p>&#8220;Left untreated or undertreated, as a result of being uninsured, will cause millions of Americans with migraine to suffer needlessly and place them at greater risk for developing more frequent migraine headaches,&#8221; said Dr. Brian M. Grosberg, director of the Inpatient Headache Program at Montefiore Medical Center and assistant professor of neurology at Albert Einstein College of <a href="http://www.nordmed.com">Medicine</a> in New York City. He was not involved in the study.</p>
<p>The lag in care may stem from the fact that the uninsured tend to seek care at hospital emergency rooms rather than a doctors&#8217; office, according to findings published in the April 13 issue of Neurology.</p>
<p>&#8220;This study is really about the need to improve the healthcare system so everybody has access to a physician in an office so they don&#8217;t have to go to an emergency department &#8212; where we know they&#8217;re not going to get the best treatment,&#8221; added the study&#8217;s senior author, Dr. Rachel Nardin, chair of neurology at Cambridge Health Alliance and assistant professor of neurology at Harvard Medical School in Boston.</p>
<p>&#8220;It&#8217;s a sad situation because we actually have pretty good treatments that can help a lot of people, but what we don&#8217;t have is a functioning healthcare system that allows us to get treatment to people who need it,&#8221; continued Nardin, who is also chair of the Massachusetts chapter of Physicians for a National Health Program.</p>
<p>According to background information in the article, some 18 percent of women and 6 percent of men suffer from migraines.</p>
<p>&#8220;Migraines are a whole-system disease. People with migraines are really out of commission,&#8221; Nardin said. &#8220;They have to lie down and sleep for several hours. People with migraines lose an average of four days of work a year, and that doesn&#8217;t include the toll it takes on them as productive members of their family or church.&#8221;</p>
<p>Meanwhile, more than 45 million Americans have no health insurance at all, and an additional 58 million are underinsured, according to the latest federal data from the National Center for Health Statistics.</p>
<p>In the study, researchers looked at results from two surveys on visits to doctors&#8217; offices, hospital outpatient visits and trips to the emergency room over a period of 11 years.</p>
<p>They estimated a total of 68.6 million such visits nationally during that time period for migraine complaints.</p>
<p>People with no insurance were twice as likely to receive inadequate therapy, either to treat or prevent migraines, as those with private coverage. Those with Medicaid were 50 percent more likely.</p>
<p>If migraine care was received in an emergency room, uninsured and Medicaid patients were almost five times more likely to receive inadequate care for their migraine.</p>
<p>&#8220;Migraine can be treated very effectively in a physician&#8217;s office,&#8221; Nardin said. &#8220;We all suffer when people need to use the emergency room because it then becomes less available for true emergencies. Migraines are a great example of where the system is not set up to get the right care in the right place at the right time. If we could do that, we would all benefit.&#8221;</p>

			<content:encoded><![CDATA[<p>Migraine sufferers may be able to get sufficient relief without turning to <a href="http://www.drugs-prescription.org">prescription drugs</a>, according to two new studies.</p>
<p>The studies, published in the latest issue of the journal Headache, conclude that <a href="http://www.drugs-prescription.org/index.php?p=drug&amp;drugBrandId=643">naproxen</a> (marketed as Aleve) and acetaminophen (Tylenol) effectively decreased or eliminated pain and reduced migraine recurrence and migraine-associated symptoms to a degree defined as a &#8220;desirable outcome&#8221; of migraine therapy by the International Headache Society.</p>
<p>Migraine headache affects as many as 28 million Americans and costs the U.S. economy an estimated $24 billion every year.</p>
<p>About three-quarters of people who suffer from migraines report more than one migraine a month. The symptoms &#8212; pain, light and noise sensitivity, nausea &#8212; can last from 4 to 72 hours and often lead to missed days from school or work.</p>
<p>Researchers from Thailand analyzed four well-designed previous studies of naproxen at doses of 500 to 825 milligrams for treatment of acute moderate to severe migraines involving 2,168 patients.</p>
<p>Led by Chuthamanee Suthisisand of Mahidol University in Bangkok, the authors concluded that naproxen effectively reduced headache intensity, pain and symptoms within 2 hours of taking it &#8211; defined by the International Headache Society as a desirable outcome.</p>
<p>When compared to other drugs known as triptans, naproxen did as well as the prescription drug frovatriptan (marketed as Frova) but did not offer the same clinical benefits as almotriptan (marketed as Axert) and zolmitriptan (marketed as Zomig).</p>
<p>However, because of side effects, not all patients can take triptans, and naproxen offers those patients a non-prescription alternative, Suthisisand told Reuters Health by email.</p>
<p>Still, the authors found that naproxen &#8220;appears to be inferior&#8221; to aspirin in treating migraines. Suthisisand said the science suggests 1,000 milligrams of aspirin is the best of several treatments that include naproxen and acetaminophen for acute moderate to severe migraine episodes, as long as patients can tolerate potential gastrointestinal side effects.</p>
<p>Although the Thai team acknowledged that the quality of studies such as theirs depends on the quality of the original studies, they said they were confident the studies they reviewed were high-quality.</p>
<p>In the second study, researchers from McNeil Consumer Healthcare, the makers of Tylenol, randomly assigned 378 migraine sufferers to either 1000 milligrams of Tylenol or a dummy pill.</p>
<p>In the 90-day trial, the researchers, led by Mary Jane Prior, found that the Tylenol group began to benefit within an hour of taking the medication. At 2 hours, 52 percent of the acetaminophen group reported that their pain was reduced to mild or no pain, compared to 32 percent of the dummy pill group.</p>
<p>The team also reported a benefit for severe pain sufferers, but they could not determine whether that was due to chance, given the small number of patients in the trial.</p>
<p>The study also found that acetaminophen offered &#8220;significantly larger&#8221; relief than placebo from nausea and noise sensitivity at 2 hours and nausea, light and noise sensitivity and functional impairment at 6 hours.</p>
<p>The study, the authors concluded, adds to earlier clinical evidence supporting acetaminophen&#8217;s use to treat migraine.</p>
<p>&#8220;When effective,&#8221; the authors wrote, &#8220;acetaminophen provides consumers with a non-prescriptive, lower cost alternative to costly <a href="http://www.drugs-prescription.org/index.php?p=search&amp;categoryId=21">prescription migraine drugs</a>.&#8221;</p>
<p>Acetaminophen is not currently approved by the FDA as a migraine treatment except as part of an aspirin or aspirin and caffeine compound. McNeil Consumer Healthcare declined to say whether they were applying to the FDA for approval of acetaminophen for use in migraine.</p>

			<content:encoded><![CDATA[<p>Older adults who take powerful <a href="http://www.orderrxpharmacy.com/pain.html">prescription painkillers</a> known as opioids face an increased risk of bone fractures, especially at moderately high medication doses, a new study finds.</p>
<p>Opioids are powerful narcotic <a href="http://www.painreliefmedicine.com">pain medications</a> that include morphine, oxycodone (Oxycontin and other brands) and hydrocodone (Vicodin and others).</p>
<p>The drugs work well against severe pain in the short term, but their longer-term effectiveness for chronic pain is less clear. Moreover, with longer use comes the risk of addiction, in addition to side effects such as nausea, constipation, dizziness and sedation.</p>
<p>That dizziness and sedation can also set opioid users up for falls, which, in older people especially, may result in serious fractures.</p>
<p>The new study, published in the Journal of General Internal Medicine, confirms the risk of fracture associated with opioids, and also shows that moderately higher drug doses further the hazard.</p>
<p>Researchers found that among more than 2,300 older adults with chronic pain, the risk of suffering a bone fracture was higher when patients were using an opioid for a prolonged period than when they were opioid- free.</p>
<p>The individuals in the study were 60 years of age or older. None of the patients was suffering from cancer-related pain. (Guidelines for treating severe cancer pain are often different than guidelines for non-cancer pain.)</p>
<p>The annual rate of fractures among study participants who were not currently using opioids was just under 4 percent, while current users showed a fracture rate of 6 percent. And among patients currently taking opioid doses of at least 50 milligrams per day, the annual fracture rate was 10 percent.</p>
<p>According to the researchers, 50 milligrams is considered to be in the moderate range for opioid doses.</p>
<p>&#8220;Some of these fractures were significant,&#8221; said senior researcher Dr.</p>
<p>Michael Von Korff, of the Group Health Research Institute in Seattle.</p>
<p>In an interview, he noted that 37 percent of fracture victims ended up in the hospital and nearly one-quarter entered a nursing home within one month of the accident.</p>
<p>The findings come at a time when long-term opioid use for non-cancer- related pain is coming under increased scrutiny.</p>
<p>About 8 million Americans are using opioids to control chronic pain, Von Korff said, yet the long-term effectiveness of the drugs is uncertain, and may vary widely from person to person. Some people find relief, while others find their pain actually worsens, Von Korff noted.</p>
<p>A report published in October by the Cochrane Collaboration, an international medical research organization, concluded that for older adults with osteoarthritis, the risks of long-term opioids may outweigh the modest <a href="http://www.painrelief.lt">pain relief</a>.</p>
<p>And in a separate study published this week in the Annals of Internal Medicine, Von Korff&#8217;s team highlights the potential for overdose among people with legitimate prescriptions.</p>
<p>The researchers found that among 10,000 patients on opioids for at least three months, 51 suffered at least one overdose, with fatal results for six. As in this latest study, higher medication doses conferred a greater risk.</p>
<p>The bottom line, Von Korff said, is that &#8220;these drugs need to be taken cautiously and under close medical supervision.&#8221;</p>
<p>The current study included 2,341 older adults who, at some point between 2000 and 2005, were prescribed opioids for at least 90 days &#8212; most commonly for chronic back pain, osteoarthritis or pain in the extremities.</p>
<p>To limit the risks of falls and other side effects, Von Korff said that patients on opioids should work with a single physician who is aware of all the medications they are taking. That will help avoid any potentially hazardous drug interactions.</p>
<p>And given the importance of dosage, Von Korff said, &#8220;never use more medication than your doctor has prescribed.&#8221;</p>
<p>He also advised opioid users who feel overly sedated or have had dizziness or falls to tell their doctors about it.</p>
